Once a site has implemented the SafeStart process and employees understand how most injuries occur, it is important to incorporate the SafeStart concepts into your incident investigations.
This is a very natural progression. The positive safety culture and common language SafeStart provides pave the way for more open and effective communications during incident discussions.
For those conducting investigations, looking at the states and errors that are involved in most incidents and injuries will shed new light on root causes (human factors) and provide a whole new perspective on accidents in your organization. The SafeStart Incident Investigation Enhancement Series does not replace your current investigation process. As the name says, it enhances it. This course will also provide specific techniques to create more effective communications with employees.
For employees, the SafeStart Incident Investigation program will teach and motivate them to conduct their own investigations, on the close calls and small errors they make, so that more significant incidents are avoided (SafeStart Critical Error Reduction Technique or CERT #2).
Traditionally, investigations are only conducted with serious incidents (infrequently for most employees). With SafeStart, everyone benefits by learning from the hundreds of minor incidents and close calls occurring every day (improving awareness and skills through frequency).
The key is to prevent similar incidents, not just the same incident, from occurring again. Often investigations result in an engineering or procedural “fix” that will prevent the exact same incident from occurring again, but won’t necessarily prevent similar incidents. For example, we need to help people get better at “moving their eyes before they move their bodies”, in addition to creating a better shelving system or protective guard.
